OMG thats a lot of cake!
I've seen worse ... but not in a long time. Holy cow. That poor pipe is probably a 15 minute smoke now. Ream it back and you'll get 1hr30min out of it. Of course, how the heck would you ream something like that? My Sr. Reamer wouldn't fit in the small opening.

Very carefully :) .. Just stick a knife in there and give it a little twist .. stick the knife in further, when the carbon starts clearing off.

Seems you'd have to pack down the pipe tobacco with a q-tip. I'd definitely start that one out with the oyster knife and ream the devil out of it until a standard reamer would fit (to even up the cake.) A wonder that one doesn't have cracks in it. I'm willing to bet there would be some very intrenched ghosts in that one but the flip side is, a cake like that takes a whole bunch of smoking. It absolutely must be a stellar smoker.
